Proof of Vaccination for 2022

ALL Parents/Spectators entering into the Arena will need to continue to complete the sign-in process and provide proof of 2 doses of COVID vaccine at entry set forth by Clearview Township and the Ontario Regulation (364/20); self-screen before you arrive at the Arena, and if you are feeling unwell please stay home!

Effective January 4, 2022 proof of vaccination will be required using the enhanced vaccine certificate with QR code and the Verify Ontario app - for anyone entering the Stayner or Creemore Arenas.  


The QR code can be used digitally or by printing a paper copy. Individuals can download their enhanced certificate with QR code by visiting here OR download the Verify Ontario App. Alternatively, residents of Clearview Township can get a free printed copy of their enhance certificate with QR code at any of the three Clearview Public Library Branches.

Out of Country/Out of Province Vaccines

Individuals who have received their vaccine(s) from another province or country are required to get the enhanced QR Code if they do not already have one. Any concerns can be directed to the SMDHU directly.
 

Medical Exemptions

Effective December 15, 2021, individuals with medical exemptions can get a QR Code to be used by contact the SMDHU directly. 

Effective January 10, 2022, the QR code for medical exemption will be required for entering the halls/recreation facilities, either digitally or printed copy will be accepted.

Lastly, we ask that you continue to kind to both our volunteers and Clearview Township staff working at the main entrance - they are simply being compliant to the Ontario Regulation requirements and disrespectful behaviour towards these individuals will not be tolerated. We expect everyone to treat one another with dignity and respect; anyone found to be in violation of the CMH Code of Conduct will face disciplinary action, up to and including removal from the facility and/or CMH organization.
