COVID19 Vaccination Requirements

Vaccination Policy for the 2021/2022 Season - Ontario Minor Hockey Association 
(2021-09-09)

It is MANDATORY that ALL eligible players, coaching staff, officials and volunteers (including CMH Executive and third-party instructors) be fully vaccinated for COVID19 for the 2021/2022 hockey season by October 31, 2021 or earlier.


NOTE: Provincial and/or Simcoe Muskoka District Health Unit regulations will supersede OMHA policy for vaccination deadlines, should they be different.          

- Fully vaccinated means fourteen (14) days after an individual has received a completed series of an accepted COVID-19 Vaccine.

- Eligible player refers to any player born in 2009 or earlier; coaching staff (head coach, assistant coach, trainer, and manager), officials and all volunteers (team volunteers, time keepers, score keepers, CMH Executive, development and/or third party instructors).

OMHA's Vaccination policy DOES NOT APPLY to hockey players born in 2010 or later and/or spectators. **Eligible Spectators may be required to show proof of COVID19 vaccination prior to entry into the Arena to the Clearview Township Staff as per Provincial and/or  local regulations

Eligible players will be required to provide proof  they have received the completed series of the COVID19 vaccination STARTING WEDNESDAY SEPTEMBER 22, 2021  
o   Acceptable documents serving as evidence of COVID19 vaccination as per OMHA include (proof of vaccination):
-  Digital or physical dose administration receipt
-  Medical records signed by a licensed health care provider indicating vaccine name and date(s) of administration
-  Government verification (i.e. government issued vaccine passport or card)

**NO PAPER COPIES of anyone’s vaccination ARE REQUIRED to be submitted. Proof of vaccination only needs to be shown to the designated CMH Staff/Executive and they will record confirmation of this action on a CMH Attendance form for pre-skates and try-outs. Once teams are created the Team Trainer will review vaccination proof for the eligible player group (i.e. team, officials, third-party instructors, etc.) and record on the CMH COVID19 Vaccination Proof form the participant full and last name, contact number OR email and indicate yes if the designated CMH Staff/Executive was shown proof of the individuals vaccination, and initial beside this section.  

Starting tomorrow - September 22, 2021 -  U13 to U18 players and coaching staff MUST come prepared to your pre-skate ice time to show proof of vaccination. NO VACCINATION PROOF = NO ENTRY 

For eligible players, coaching staff, CMH Executive, officials, third-party instructors - failing to comply with the OMHA Vaccination Policy and/or Provincial/Local Regulations, will result in suspension from the CMH Organization.

For families and fans
; failure to comply with the Provincial/Local Regulations set forth by Clearview Township will result in denied entry into the Arena and/or Clearview Township has informed CMH they will invoke locked doors and refusal to admit any/all spectators into the Arena.
