Digital Health Screens information

Many of you will have received an email inviting you to create a log-in for the SprotsHeadz system.  We will be using this program for the health screens required for ALL players and coaching staff prior to every ice session. Bonus! this program is linked to our CMH website and will automatically sync to the ice schedule for your division/group team (i.e. U9 Group 2), alerting you 24 hours before that you need to complete the health screen! 

Once you are registered you will see the team name displayed on your main page – this will look different depending if you use the website or the app. At this time NO health screens are required as we don’t start until Tuesday October 13! For those starting on Tuesday (U9 Group 2; U13 Group 2; U18 Group 2) you will be able to log-on to SportsHeadz 24hours before you ice time, identify if you are attending:
-          If you select NO, you are done and the coaches will see you are not attending.
-          If YES; it will prompt you to complete the health screen **be sure to click SUBMIT at the end of questions!!!

Coaching staff will be able to see who is attending, who has completed a screen and who hasn’t enter anything yet! 


Arrive at your ice time and forgot to complete the health screen? No problem, you can access the health screen from our website (go to your team page) OR download the SportsHeadz app on Google Play OR in the App store! 

**REMEMBER you can only be admitted into the arena 15 minutes before ice time and arena doors will lock at the scheduled ice time.
